There is likely to be a fee for reports and letters, please discuss what is required with our receptionists who will provide a form for completion.

We can accept payments via cash, cheque (made payable to Southgates Medical Centre) or by card (at Southgates only).

If you wish to pay by bank transfer, please let us know so that we can forward the details to you. Once payment has been received along with completed form we will pass on your request to a clinician, some requests might not be possible in which case we will refund any fees paid.
