You must live within our practice boundary, if you wish to register as a new patient.

You will need to complete our New Patient Registration form, you will also need to request an appointment to attend for a blood pressure check if aged over 45.

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. 

When you register, you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
